PROOF OF PURCHASE Proof of purchase receipt(s) must consist of both an itemized, dated sales receipt showing all products purchased, qualified product item numbers, prices paid for qualifying products, and the authorized retailer's name, city and state. Handwritten receipts will not be accepted. Order acknowledgements, packing slips and purchase order copies are not valid. Proof of purchase/UPC bar code labels must be the original code from qualifying product packaging. The UPC bar code labels must be cut from the product packaging so the cardboard or plastic backing is completely removed from the packaging, and include part numbers with bar codes intact.
